How to access the Hospice Care Note within EMIS
Please follow the steps below to access the Hospice Care note within EMIS:
Log into EMIS Web, Open the patient for whom you wish to create a new Hospice Care Note for.
In the summary view click on View -> My Record highlighted in red on the left hand side (see Figure 1)

When the menu expands, select Black Pear Core from the menu (highlighted in Red - Figure 2)

Once Black Pear Core has been selected the below view should now be displayed. Click on Launch Black Pear Core to continue (Figure 3)

The application then loads Microsoft Edge in a separate browser window. The patient details from EMIS will be loaded with the option to Create a Hospice Care Note (Figure 4)

How to access the Hospice Care Note within SystmOne
Log into SystmOne. Open the patient for whom you wish to create a new Hospice Care Note for.
In the Patient record View, click on the Black Dot (highlighted in red below) to open (Figure 5)

The application then loads Microsoft Edge in a separate browser window. The patient details from SystmOne will be loaded with the option to Create (Figure 6)

Important Note
Once you have entered information into the Hospice Note either from launching from EMIS or SystmOne and saved the changes, please ensure you close the Microsoft Edge browser window, by clicking on the cross (see figure 7 below)

How to complete a Hospice Note
Once the patient has loaded, please click the Create button on right hand side to create a new Hospice Care Note (Figure 8)

Once the Hospice Note has loaded, enter the information you wish to note into the Special Patient Note box, ensuring the box is ticked for ‘Patient has been informed that this Basic Note will be shared’ (see Figure 9 below).

Green circles with ticks indicate where fields in the subform have been populated. Information can be checked either by clicking on the Green circles or by clicking on the Black arrows (Figure 10).

When you have entered information into the Special Patient Note section, please click Save on the section view of the plan (Figure 11)

Once complete you should then see the below screen – Click on Save again (Figure 12)

You will then see the system saving the Hospice Care Note to the GP record and pyrusCloud.
Wait until you see both sections turn Green (Figure 13)
Once complete Click on Back to active session to return the patient page, once both sections have turned green

You will then be taken back to the main screen, where another Hospice Care Note can be created.

To see the Hospice Care Note information that has written back into SystmOne. Close the Microsoft Edge browser, which will return you to the patient record within SystmOne. Click on Save on the patient that you are currently in, then search for the patient again, navigate to the Read Code Journal View and the Hospice Note will appear (see figure 15)

To see the Hospice Care Note information that has written back into EMIS, Close the Microsoft Edge browser, which will return you to the patient record within EMIS. Refresh the page, then navigate to Consultation View and the Hospice Note information will appear. (see Figure 16)
